Model-informed precision dosing platform for intravenous vancomycin in adults, developed at the Department of Pharmaceutical Care, King Abdulaziz Medical City – Jeddah (MNGHA), under IRB NRJ24/006/12.

MAP BayesianFull dose-history superposition; proportional residual error; Nelder–Mead with restarts; posterior SD from the Hessian (Laplace approximation); 90% CI on CL and AUC₂₄. ConcordancePopPK and ridge a priori clearances compared; <20% concordant, 20–40% moderate, >40% discordant. Weighted prior uses inverse external-RMSE weights (PopPK 0.47, ridge 0.53).

Clinical use

Decision support only. Recommendations require review by a clinical pharmacist. Target AUC₂₄ 400–600 mg·h/L assumes MIC ≤ 1 mg/L (Rybak et al., 2020). Reassess renal function and re-run MAP after any change in SCr > 25%.
